Needs assessment & advice

Every collaboration begins with a conversation in your home. A registered nurse carefully assesses your nursing and support needs – taking into account your health, your daily life and your wishes. This results in a care plan that suits you.

What is included

  • Needs assessment at home

    Together we establish where you need support and what you would like to keep doing yourself – in your familiar surroundings.

  • Individual care plan

    We agree goals and measures with you and review them regularly. If your situation changes, we adjust the plan.

  • Advice and guidance

    We show you and your family how to use aids safely or carry out certain tasks yourselves – such as injecting insulin.

  • Coordination

    We liaise with your GP, hospitals and other specialist services so that everyone involved is well informed.

  • Aids and equipment

    We advise you on which aids will make your daily life easier and help you organise them.

Funding

Assessment, advice and coordination prescribed by a doctor (Art. 7 para. 2 let. a KLV) are compulsory benefits of health insurance under the KVG. The deductible, co-payment and patient contribution according to the rate list apply.
